About The Position
We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in four (4) of our adult residential facilities in South Bend, IN. The role involves ensuring that each individual receives quality, individualized health care management, resulting in their optimal health. You will conduct routine site visits, check vitals, perform assessments, counsel on medication administration, and document progress. Additionally, you will act as a health/medical resource for staff, train staff in the medical arena, conduct audits and reviews of medications, and ensure compliance with state requirements and organizational policies regarding medical care and records.
