Requirements

Proof of current license as an LPN or LVN in good standing from the state(s) of practice
Current CPR certification
Ability to work in a home-like setting.
Be able to bend stoop, push, pull, sit, and walk for periods of time
Able to utilize proper body mechanics while lifting 50 pounds
Knowledge of individual service plans, including understanding of client rights and health needs
Must have completed post-secondary coursework to facilitate licensure as an LPN or LVN

Responsibilities

Delivering attentive and personalized care following each client's unique care plan
Monitoring and analyzing clients' vital signs, promptly reporting any notable findings to the healthcare team
Administering medications, feedings, oxygen, and specialized care according to individual care plans, ensuring each client's specific needs are met.
Collaborating with other healthcare professionals such as Physical Therapists (PT), Occupational Therapists (OT), and Speech-Language Pathologists (SLPs) to ensure comprehensive care routines are established and maintained.
Maintaining accurate documentation of all nursing actions, effectively communicating and documenting progress and outcomes for established goals.
