About The Position

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) setting in Youngsville, PA. This role involves providing high-quality nursing care to residents in a skilled nursing facility while collaborating closely with Registered Nurses, physicians, and interdisciplinary care teams.

Requirements

• Active Pennsylvania LPN license (required)
• Minimum 1 year of LPN experience , preferably in Long-Term Care or Skilled Nursing
• BLS certification (required)
• Strong clinical judgment, organizational, and documentation skills
• Ability to work variable shifts and 12-hour schedules
• Compassionate, patient-centered approach to care

Responsibilities

• Provide direct nursing care to residents in a long-term care environment
• Administer medications and treatments in accordance with physician orders
• Monitor, assess, and document residents’ conditions; promptly report any changes
• Assist in developing and implementing resident care plans
• Maintain accurate and timely medical records
• Support residents with activities of daily living (ADLs) as needed
• Ensure compliance with facility policies, safety standards, and state regulations
• Communicate effectively with residents, families, and healthcare team members
