About The Position

The Licensed Practical Nurse (LPN/LVN) provides direct nursing care to individuals with developmental and/or long-term care needs under the supervision of a Registered Nurse (RN). This role involves administering medications, performing clinical procedures, monitoring patient conditions, and supporting daily living activities in a residential care setting.

Requirements

  • Active New York State LPN license (must be in hand)
  • Ability to work mandated shifts and float as required
  • Experience in long-term care, developmental disabilities, or residential settings preferred
  • SCR and SEL clearance required prior to start (processing may take 2–4 weeks)

Responsibilities

  • Administer medications and injections as prescribed
  • Perform tube feedings, hang IVs, and provide colostomy care
  • Collect blood samples (if venipuncture certified)
  • Monitor patient condition and report changes to RN or physician
  • Take and document vital signs (temperature, pulse, respiration, blood pressure)
  • Obtain specimens for laboratory analysis
  • Prepare and apply simple dressings
  • Assist with bathing, feeding, hygiene, and activities of daily living (ADLs)
  • Support residential care duties including bed-making, linen changes, lifting, and repositioning patients
  • Maintain patient safety and ensure continuity of care at all times
