About The Position

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate LPN/LVN to deliver high-quality patient care in a Long-Term Care (LTC) and Rehabilitation setting. The ideal candidate will have experience in skilled nursing environments and the ability to manage a diverse patient population while maintaining exceptional care standards.

Requirements

• Active LPN/LVN license required
• Minimum 1 year of LPN experience in LTC setting required
• BLS certification required
• Strong communication, clinical, and organizational skills

Nice To Haves

• Experience in a Skilled Nursing Facility or Nursing Home preferred
• Familiarity with EMR systems preferred

Responsibilities

• Provide nursing care to LTC and rehabilitation patients in accordance with established protocols
• Monitor and document patient vitals, including blood glucose levels
• Administer medications and treatments as prescribed
• Float between multiple units within the facility as needed
• Maintain accurate and timely documentation using an EMR system
• Respond promptly to patient needs while ensuring proper care ratios
• Collaborate with interdisciplinary teams to support optimal patient outcomes
• Adhere to all facility policies, procedures, and safety guidelines
