About The Position

The Licensed Practical Nurse (LPN) is responsible for providing high-quality, compassionate nursing care to patients in their homes as part of a primary care practice. Working closely with the provider, the LPN will perform a variety of nursing duties, including evaluations, documentation, medication management, wound care, and patient education. The LPN will serve as a critical link between the practice and the patients, ensuring seamless coordination of care and promoting positive health outcomes.

Requirements

  • Current and valid LPN license in the state of Oregon.
  • Experience providing direct patient care, preferably in a home health or primary care setting preferred.
  • Strong clinical skills in areas such as wound care, medication management, and patient education.
  • Proficiency in phlebotomy techniques and ability to collect laboratory samples as ordered by provider.
  • Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and the healthcare team.
  • Proficient in electronic medical record (EMR) systems and data management.
  • Commitment to providing high-quality, patient-centered care and promoting positive health outcomes.
  • Valid driver's license and reliable transportation to travel to patient homes.

Responsibilities

  • Conduct comprehensive in-home patient evaluations, including vital signs, symptom evaluation, and monitoring of chronic conditions.
  • Administer prescribed medications, injections, and other treatments as directed by the supervising provider.
  • Collect laboratory samples, assist with explaining procedures, safe and sanitary patient laboratory sample handling.
  • Provide skilled nursing care for acute and chronic wounds, including dressing changes and application of topical treatments.
  • Educate patients and their families on disease management, medication administration, and health promotion.
  • Collaborate with the provider to develop and implement individualized care plans for each patient.
  • Maintain detailed patient records and documentation in accordance with regulatory requirements.
  • Coordinating referrals and make upcoming appointments for the patient on site, providing patient with upcoming appointment schedules.
  • Communicate effectively with the primary care team, patients, and their families to ensure continuity of care.
  • Participate in team meetings and clinical case reviews to discuss patient progress and coordinate care.
  • Maintain a high level of professionalism, empathy, and cultural sensitivity when interacting with patients.
  • Provide temporary administrative, reception, and scheduling support during initial phases of the practice start-up to ensure smooth operational launch and seamless client interactions.
