As a Hospice LPN, you’ll provide skilled, compassionate nursing care in patients’ homes while supporting families through one of the hardest seasons they will ever face. Requirements

  • Graduate of an accredited practical or vocational nursing program
  • Current, unrestricted LPN license in the state of New Mexico
  • Minimum of one year of clinical nursing experience (home health, hospice, or acute care preferred)
  • Current CPR (BLS) certification
  • Strong clinical judgment, time management, and communication skills
  • Ability to work independently in the field while staying closely connected to the care team
  • Emotional steadiness, compassion, and reliability — especially in difficult situations
  • Valid driver’s license, reliable transportation, and current auto insurance

Nice To Haves

  • Prior hospice experience strongly preferred

Responsibilities

  • Provide skilled hospice nursing care under the direction of a Registered Nurse and the established plan of care
  • Administer medications and treatments in accordance with physician orders and agency policies
  • Monitor and document changes in patients’ physical, emotional, and psychosocial status, reporting significant findings promptly
  • Educate patients and caregivers on comfort measures, medications, and what to expect throughout the hospice journey
  • Collaborate closely with RNs, social workers, aides, chaplains, and other team members to ensure coordinated, patient-centered care
  • Maintain accurate, timely clinical documentation in the EMR in compliance with CareM standards
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement activities
  • Navigate complex family dynamics with professionalism, empathy, and healthy boundaries
