Requirements

  • Active LPN/LVN license in the state of practice
  • Current CPR certification
  • Minimum of one (1) year nursing experience (home health, community, or med-surg a plus)
  • Strong clinical judgment, communication, and organization skills
  • A patient-first mindset and passion for meaningful care
  • Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

  • Provide direct patient care in the home under the supervision of a Registered Nurse
  • Execute individualized care plans and support positive patient outcomes
  • Educate patients and caregivers on disease management and self-care
  • Document care accurately and timely in the EMR
  • Collaborate with physicians and interdisciplinary teams to ensure coordinated care
  • Build trusted relationships with patients, families, and care teams
