Requirements

Active LPN/LVN license in the state of practice
Current CPR certification
Minimum of one (1) year nursing experience
Strong clinical judgment, communication, and organization skills
A patient-first mindset and passion for meaningful care
Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

Experience in home health, community, or med-surg nursing

Responsibilities

Provide direct patient care in the home under the supervision of a Registered Nurse
Execute individualized care plans and support positive patient outcomes
Educate patients and caregivers on disease management and self-care
Document care accurately and timely in the EMR
Collaborate with physicians and interdisciplinary teams to ensure coordinated care
Build trusted relationships with patients, families, and care teams
