About The Position

Join our compassionate team providing skilled nursing care in patients’ homes. We are seeking dedicated LPNs who value flexibility, teamwork, and excellent patient outcomes.

Requirements

• Current LPN license in Nevada (required).
• Minimum 1 year of home health nursing experience (required).
• Strong communication and teamwork skills.
• Ability to work independently and manage a flexible schedule.

Responsibilities

• Provide nursing visits according to the supervising RN’s plan of care.
• Maintain visit frequency and communicate changes as needed.
• Perform home safety and environmental checks; report concerns to the RN.
• Prepare equipment and perform treatments using proper aseptic technique.
• Teach patients and families self-care; reinforce the HHA plan of care.
• Collaborate with the RN and care team, participating in case conferences.
• Submit accurate, timely documentation each week.
• Recommend community resources to support safe care at home.
• Attend staff meetings, contribute to quality improvement, and mentor new team members.
