About The Position
Lake Prince At Home, Home Health and Hospice is seeking, LPNs for our growing team. Lake Price Woods at Home is a non-profit organization that prides themselves on serving the community at EveryAge. POSITION SUMMARY: Under the direction of the RN Case Manager, provides nursing services/ patient care activities and accommodation services in accordance with standards, regulatory guidelines, and licensure requirements. Follows the plan of care while providing services. Communicates with Manger changes and collaborates with patient/family member/physician or Nurse Practitioner as needed or directed.
Requirements
- Unencumbered Virginia license as a Licensed Practical Nurse.
- Graduate of an accredited school of nursing.
- One year of nursing experience, with Home Health.
- Ability to Commute: Suffolk, VA 23434 (Preferred)
