About The Position

SummitWest Care is hiring a Licensed Practical Nurse (LPN) for our home health team-where you can focus on one patient at a time, build meaningful relationships, and work a consistent weekday schedule. As a nonprofit home health agency, we prioritize quality care, realistic workloads, and respect for our nurses-because great patient care starts with supported staff.

Requirements

• Active Colorado LPN license (unrestricted), IV Certification a plus!
• Ability to work independently and manage your schedule in the field
• Strong communication and patient-centered care approach
• Comfort with documentation and basic technology
• Valid driver's license, reliable vehicle, and auto insurance

Nice To Haves

• Home health or community-based care experience preferred (we will train the right candidate)

Responsibilities

• Provide skilled nursing care in a home health setting under physician direction
• Perform in-home visits, including treatments, monitoring, and follow-up care
• Administer medications and treatments within LPN scope of practice
• Observe and report changes in patient condition to the RN and care team
• Educate patients and families on medications, disease management, and safety
• Document visits in Homecare Homebase (HCHB) – training provided
• Collaborate with an interdisciplinary team (RNs, therapists, caregivers)
• Travel locally within Mesa County (Grand Junction, Fruita, Clifton, Palisade)
