Inspire health. Serve with compassion. Be the difference. The position involves assessing, developing, implementing, and evaluating nursing care for assigned patients. The role is responsible for coordinating with other disciplines and agencies involved in the patient's care.

Requirements

  • Completion of an LPN program recognized by the SC State Board of Nursing.
  • One (1) year experience as an LPN.
  • Experience in Home Health preferred.
  • In lieu of one (1) year of LPN experience, will accept successful completion of the Prisma Health Nurse Residency Program or a comparable residency program.
  • In lieu of completion of a LPN program, will accept program equivalency recognized by the SC State Board of Nursing.
  • In lieu of one (1) year of LPN experience, will accept candidates enrolled in an accredited LPN to RN transition program.

Responsibilities

  • Implements the nursing care plan for each patient.
  • Provides nursing services, treatments, and diagnostic and preventive procedures according to the plan of care as assigned.
  • Initiates preventive and rehabilitative nursing procedures as appropriate.
  • Prepares equipment and materials for treatments.
  • Assists the RN in performing specialized procedures.
  • Observes aseptic technique as required.
  • Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
  • Evaluates with the registered nurse the effectiveness of the LPN's nursing service to the patient and family under the guidance of the registered nurse.
  • Maintains accurate and complete records of observations, treatments and care of patient.
  • Prepares clinical and progress notes.
  • Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
  • Assists the patient in learning appropriate self-care techniques.
  • Participates in patient care conferences and discusses the need for involvement of other members of the health team.
  • Submits any changes in schedule to clinical manager on a daily basis.
  • Understands and adheres to established policies and procedures.
  • Attends staff meetings, patient care conferences, and in-services as scheduled.
  • Participates in medical record audits as assigned.
  • Performs other duties as assigned.
