About The Position

We are seeking a caring, reliable, and motivated Licensed Practical Nurse (LPN) to join our Home Health Nursing team. As an LPN in home health, you will provide direct patient care, support care plans designed by RNs and physicians, and bring comfort and clinical expertise directly into patients' homes. Requirements

• Current Licensed Practical Nurse (LPN) license in Indiana.
• Previous nursing experience preferred; home health experience a plus (training available).
• Strong communication, organization, and time-management skills.
• Compassionate, dependable, and dedicated to patient care.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes under the supervision of an RN.
• Monitor and document patient conditions, vitals, and progress notes.
• Administer medications and treatments as prescribed.
• Educate patients and families on care, medications, and healthy lifestyle practices.
• Collaborate with the care team (RNs, physicians, therapists) to ensure quality outcomes.
• Maintain compliance with all healthcare regulations and company standards.
