We are seeking a caring, reliable, and motivated Licensed Practical Nurse (LPN) to join our Home Health Nursing team . As an LPN in home health, you will provide direct patient care, support care plans designed by RNs and physicians, and bring comfort and clinical expertise directly into patients' homes. Requirements

• Current Licensed Practical Nurse (LPN) license in Indiana.
• Strong communication, organization, and time-management skills.
• Compassionate, dependable, and dedicated to patient care.
• Valid driver's license and reliable transportation.

Nice To Haves

• Previous nursing experience preferred; home health experience a plus (training available).

Responsibilities

• Provide skilled nursing care to patients in their homes under the supervision of an RN.
• Monitor and document patient conditions, vitals, and progress notes.
• Administer medications and treatments as prescribed.
• Educate patients and families on care, medications, and healthy lifestyle practices.
• Collaborate with the care team (RNs, physicians, therapists) to ensure quality outcomes.
• Maintain compliance with all healthcare regulations and company standards.
