Requirements

• Active and unexpired Licensed Practical Nurse (LPN) license in New York.
• MediClear or equivalent healthcare credential required.
• Completion of an accredited nursing program.
• In -depth knowledge of infection control measures, patient care protocols, and healthcare regulations.
• Strong communication and teamwork skills.
• Ability to manage multiple priorities while maintaining accuracy and a high quality of care.

Responsibilities

• Provide direct patient care while maintaining the highest standards of safety and infection control.
• Administer medications and perform treatments as prescribed.
• Accurately document patient admissions, transfers, discharges, and ongoing care in electronic medical records.
• Monitor patient conditions, communicate changes to physicians, and adjust care plans as needed.
• Educate patients and families about therapies, rehabilitation regimens, and health adjustments.
• Collaborate with physicians, RNs, and interdisciplinary teams to ensure optimal patient outcomes.
• Uphold confidentiality standards and adhere to Department of Health regulations and facility policies.
