About our Team Highpoint Health Partners offers primary care services dedicated to preventing, diagnosing and treating illnesses that affect patients of all ages. A primary care provider serves as the first point of contact for patients, offering continuity of care and referrals to specialists when necessary. The goal of primary care is to keep individuals healthy and address their medical concerns in a holistic manner. Primary care services typically include preventive care, such as vaccinations and screenings , as well as management of chronic conditions like diabetes and hypertension. Services include: Annual wellness exams and checkups Sick visits Preventive screenings Flu shots and other immunizations Minor injuries or accidents Medication management Allergy treatment Healthy lifestyle education Ongoing care for chronic conditions Telehealth now available How you’ll contribute As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice. Reports to: The Director of their assigned department What We're Looking For Under the direction of an RN or Med-Surge Charge Nurse, contributes to the assessment of patients by collecting data for analysis by an RN. Performs plan of care interventions, including specimen collection and wound care. May administer medication. Documents patient care given. Assists RN with development of plan of care. Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families. Works as an advocate for the physical and emotional well-being of the patient. Qualifications & Requirements Graduate of a Practical Nursing program.
