Requirements

  • Associate’s degree or Diploma in LPN
  • Current LPN to practice in Kentucky
  • At least 1 year of LPN experience (acute care experience preferred).
  • BCLS

Responsibilities

  • As supervised and delegated by a registered nurse, provides direct patient care to patients in accordance with applicable scope and standards of practice.
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.
  • Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.
  • Performs plan of care interventions, including specimen collection and wound care.
  • Administers medication per scope of practice.
  • Documents patient care given.
  • Assists RN with development of plan of care.
  • Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
  • Works as an advocate for the physical and emotional well-being of the patient.
  • Adherence to all core measure initiatives, patient safety goals, and NQF Safe Practices.
  • LPNs who have the additional skill set and competencies to work in an expanded practice team nursing model must complete annual education timely, maintain applicable certifications/competencies, and have a satisfactory performance appraisal. Absences in excess of three in a rolling 365-day period will result in the employee becoming ineligible for the LPN Expanded Practice role; at that time, the employee will be moved into a regular LPN position and the base rate of pay adjusted to the employee’s status. The employee may reapply for LPN Expanded Practice role after 6 months.
