As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice. Reports to the Director or Manager of Department. LPNs who have the additional skill set and competencies to work in an expanded practice team nursing model must complete annual education timely, maintain applicable certifications/competencies, and have a satisfactory performance appraisal. Absences more than three in a rolling 365-day period will result in the employee becoming ineligible for the LPN Expanded Practice role; at that time, the employee will be moved into a regular LPN position and the base rate of pay adjusted to the employee’s status. The employee may reapply for LPN Expanded Practice role after 6 months.

Requirements

  • Education: Graduate of accredited LPN program.
  • License/Certifications: Current LPN license in state of practice.
  • BCLS required.
  • Handle with Care preferred.

Responsibilities

  • Contributes to the assessment of patients by collecting data for analysis by an RN.
  • Performs plan of care interventions, including specimen collection and wound care; may administer medication.
  • Documents patient care provided.
  • Assists RN with development of plan of care.
  • Assist patients with daily living activities including personal hygiene, elimination, nutrition, and ambulation.
  • Monitors, documents, and communicate patient status and responses to interventions with RN, physician, patient, and family.
  • Advocates for the physical and emotional well-being of the patient.
  • Participate in performance improvement and continuing education.
  • Provides quality patient care and works flexible shifts/units as needed.
  • Maintains regular and reliable attendance.
  • Performs other duties as assigned.
