About The Position

The Licensed Practical Nurse (LPN) is a key member of the interdisciplinary team to deliver patient-centered acute nursing care to individuals with a variety of medical issues or who are preparing for or recovering from surgery. The LPN as a collaborative team member in partnership with and under the supervision of the registered nurse. Schedule 3 - 12-hour shifts/week from 700pm to 7:30am. Rotating Weekends.

Requirements

• Graduate of an accredited School of Practical Nursing.
• Minimum of one (1) year experience within a Rehabilitation or acute setting; new graduates considered based on student clinical experiences.
• LPN - Licensed Practical Nurse - State Licensure in the District of Columbia required.
• CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required.

Responsibilities

• Organizes, prioritizes, and cares for groups of patients under the direction of the Registered Nurse.
• Establish effective relationships with the patient/family to facilitate the development of a patient-centered care plan unique to the individual's care needs.
• Collaborates with other healthcare team members to implement a plan of care and education to maintain and restore health and prevent complications.
• Provide patient and family education about measures of health promotion, maintenance, and restoration to promote comfort.
• Provides direct nursing care to patients as assigned which includes collecting assessment data, administration of medications, and performing activities of daily living including bathing, mouth care, feeding patients, range of motion, walking patients and transporting patients.
• Performs sterile and non-sterile treatments in accordance with Hospital policy, including tube feedings, ostomy care, Foley catheter insertion/care, IV management, dressing changes, and wound care.
• Reports changes in patient status to the RN and members of the health care team as they occur.
• Collaborates with the RN on an ongoing basis regarding effective nursing interventions and patient care.
• Provides direction and supervision of the patient care associates.
• Serves as an advocate for the patient and family.
• Participates in the orientation of staff members/students as designated by the Nurse Manager.
• Contributes to the implementation of the unit objectives and performance improvement activities.
• Maintains a safe and orderly patient/unit environment.
• Fosters a healthy work environment within the practice setting and profession.
• Participates in self and peer review.
• Promotes professional engagement.
