  • Reviews and clears HCHB coordination notes and processes administrative task workflows in an accurate and timely manner.
  • Obtains orders from physicians, including verbal, written and faxed orders, and enters into appropriate system for CTM review and approval.
  • Monitors patient hospitalization list, confirms status, and coordinates appropriate resumption of care.
  • Discharges patients in inpatient facility at end of episode (EOE)
  • Obtains patient-related attachment renewal.
  • Participate in High-Risk Meetings and ensure that the best provision of services is offered.
  • Available to provide direct care, as needed.
  • Answers telephone calls from patients, patient representatives, and medical professionals and communicate messages to the appropriate party or parties.
  • Provides clerical support to patients such as but not limited to scheduling follow-up appointments for patients (as requested), providing health summaries to case managers, obtaining pre-authorizations, and addressing patient questions or concerns.
  • Graduate of an accredited school of professional nursing
  • Current Certified Licensed Vocational Nurse or Licensed Practical Nurse
  • 1 year of home care or hospice experience
  • Able to seek patient feedback, advocate for patients, understanding of patient needs, and managing patient expectations
  • Able and willing to travel up to 50% as needed to support business and patient needs.
  • Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
  • Valid driver’s license and insurance and reliable transportation to perform job tasks
