About The Position

The Licensed Practical Nurse (LPN) provides healthcare services to patients in our Center for Behavioral Health. They collaborate with and support team members to ensure a patient-focused approach to care. As part of the essential functions of this role, the Licensed Practical Nurse:

Requirements

  • current LPN license in New York
  • current BLS certification
  • candidate must be able to communicate clearly and professionally in writing and verbally
  • candidate must demonstrate familiarity with computers and MS Office software (Outlook, Word) and electronic charting/medical records systems (Medent preferred)
  • candidate must demonstrate personal responsibility and integrity and ability to handle Protected Health Information (PHI) in a manner consistent with HIPAA
  • awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential

  • minimum of one (1) year of experience working in a behavioral health setting preferred

Responsibilities

  • Provides outstanding customer service while maintaining a compassionate, positive, and respectful attitude toward people who use drugs
  • Assesses, plans, implements, and evaluates care for patients under their scope of practice. Prepares patients for provider visits by taking vital signs, history and assessing risk factors
  • Collects and records health information findings of physical and developmental assessments into patient records with timeliness and accuracy; accesses and retrieves medical records via HEALTHeLINK
  • Responsible for screening all new and returning patients by completing an intake process; comprehensively documents intake encounters and health history into our EMR system
  • Facilities lab and diagnostic tests, including injections, urine, and oral toxicology at the request of the provider
  • Obtains insurance prior authorizations for referrals, imaging, and medications prescribed by provider(s) and prepares medication refill prescriptions under supervision of prescribing provider
  • Assists with paperwork as needed
  • Collaborates with Harm Reduction Center staff to identify needs and refer patients to additional services, including mental health and/or substance use counseling, care coordination, primary and specialty medical care, inpatient or detox programs, housing, and HIV and HCV testing
  • Maintains exam rooms according to protocols; cleans rooms and equipment between patient exams to ensure patients' comfort and compliance with infection control standards; ensures all necessary supplies are stocked and equipment is functioning properly
  • Assists providers, and registered nurses with procedures if needed
