About The Position

The Licensed Practical Nurse (LPN) – Behavioral Health provides direct nursing care to residents in a dementia care unit within a skilled nursing facility. The LPN supports residents with cognitive impairments and behavioral health needs by monitoring patient conditions, administering treatments, and implementing behavior management strategies. This role requires strong clinical judgment, effective communication, and the ability to manage challenging behaviors in a safe and compassionate environment.

Requirements

  • Active Licensed Practical Nurse (LPN) license in the state of New York.
  • Minimum 2 years of nursing experience , preferably in behavioral health or long-term care settings.
  • BLS (Basic Life Support) certification.
  • Experience working in locked behavioral health or dementia care units .
  • Knowledge of behavior management techniques and de-escalation protocols .
  • Experience with blood glucose monitoring, intake and output tracking, and vital sign monitoring .
  • Strong communication, observation, and patient care skills.

Nice To Haves

  • Experience in skilled nursing facilities or nursing homes .
  • Familiarity with Epic electronic medical record (EMR) system .
  • Experience managing assaultive or challenging patient behaviors in behavioral health settings.

Responsibilities

  • Provide direct nursing care to residents with dementia and behavioral health conditions.
  • Monitor patient health status, including vital signs, blood glucose levels, and overall condition.
  • Administer medications and treatments as prescribed by physicians.
  • Utilize behavior management and de-escalation techniques to address agitation or aggressive behaviors.
  • Maintain accurate records of patient intake and output, observations, and care provided.
  • Assist with specimen collection, labeling, and documentation.
  • Implement isolation precautions and infection control protocols as required.
  • Collaborate with physicians, registered nurses, and care team members to ensure coordinated patient care.
  • Monitor and report changes in patient condition to the healthcare team promptly.
  • Support residents with daily care needs while promoting safety, dignity, and comfort.
