About The Position

LindenGrove Mukwonago, an Illuminus Community, is hiring for a Licensed Practical Nurse (LPN). We offer a continuum of care which includes assisted living, memory support, and short-term rehabilitation and long-term skilled nursing care to older adults. We are passionate about offering holistic, transformative care and helping those in need rediscover their light within. Our LPN provides prescribed medical treatment under the general supervision of a registered nurse. Requirements

  • Current Licensed Practical Nurse credentials and license in the State of Wisconsin.
  • Active CPR certification.
  • Commitment to quality outcomes and services for all individuals.
  • Ability to relate well to all individuals.
  • Ability to maintain and protect the confidentiality of information.
  • Ability to exercise independent judgment and make sound decisions.
  • Ability to adapt to change.

Responsibilities

  • Maintains and actively promotes effective communication with all individuals.
  • Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
  • Responsible for on-call services, as assigned.
  • Leads, guides and develops certified nursing assistants and resident assistants in the provision of quality care.
  • Supervises the duties of certified nursing assistants and resident assistants.
  • Performs medical treatments and reports status to the registered nurse.
  • Reports resident changes in condition promptly to the registered nurse.
  • Performs all necessary related functions, such as blood pressure checks and pulses in conjunction with the medication pass as needed.
  • Participates in following and updating plans of care.
  • Promotes resident centered care.
  • Performs other duties, as assigned.
