The Licensed Practical Nurse (LPN) - Ambulatory Care, Float Pool position supports Bassett Medical Group (BMG) by providing clinical coverage across a network of outpatient clinics. The Bassett Medical Group is comprised of full-time physicians and advanced practice clinicians who provide primary and specialty care at the Bassett Clinic, Bassett Medical Center in Cooperstown, and over two dozen community-based primary care centers across eight counties. LPNs in this float role travel frequently to various BMG sites-including Cooperstown, Norwich, Oneonta, Herkimer, Cobleskill, Delhi, and Little Falls-to ensure consistent patient care coverage. This position is eligible for mileage reimbursement and typically follows a Monday-Friday schedule from 8:00 a.m. to 5:00 p.m., with occasional Saturday coverage as needed. Other opportunities for flexibility of schedule assignments are available.

Requirements

  • Graduate of a Practical Nursing program, required
  • Current NYS LPN License and registration as a Practical Nurse or a valid interim permit, required

Responsibilities

  • Provides direct and indirect nursing care individualized to the patient's needs.
  • Collects and records relevant patient health data accurately
  • Evaluates health data to identify patient problems
  • Implements and documents specified interventions
  • Identifies deviations from expected patient outcomes
  • Adheres to approved, research-based practice standards
  • Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures
  • Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures
  • Advocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights.
  • Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity
  • Assures that patients have informed consent signed for indicated procedures
  • Maintains current licensure, certification, and competencies.
  • Provides proof of current BLS validation to supervisor.
  • Completes required unit credentialing according to departmental policy
  • Provides to supervisor evidence of completion all required annual training and competency validation
  • Practices within the New York State-defined LPN scope of practice 100% of the time
  • Acquires and maintains current knowledge and skills in his/her area of practice.
  • Performs other duties as requested and observed by supervisor or manager
