Licensed Practical Nurse (LN II)
About The Position
We are seeking an LPN to work with patients in a clinical care environment. We are looking for an LPN who demonstrates great work ethic, dependability, ability to work with a team, and proficiency in patient documentation, both electronic and written. The job responsibilities will include, but are not limited to: Access and triage patients, medication reconciliation, coordinate patient care with other departments, phoning in prescriptions, medication administration, scheduling procedures, documentation. Communicate with patient care and collaborate with all the members of the healthcare team. The nurse will serve as a patient advocate by facilitating education and access to community resources. Case Management experience preferred. Our office hours are Monday through Friday, 8:00 am to 5:00 pm. No weekends or holidays are required. An orientation will be provided based on your experience and level of competency.
