About The Position

"Navitas Healthcare, LLC" is seeking a dedicated Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN) to provide quality nursing care in a long-term care setting. The ideal candidate will support patients with medical-surgical, orthopedic, dementia, and memory care needs while ensuring high standards of patient care.

Requirements

• Active LPN/LVN license (New York) required
• Minimum 1+ year of experience required
• BLS certification required
• Experience in long-term care preferred
• Familiarity with EMR systems (PointClickCare preferred)
• Strong clinical and patient care skills
• Ability to work in a fast-paced LTC environment

Responsibilities

• Provide direct patient care in a long-term care setting
• Administer medications and treatments as prescribed
• Monitor patient conditions and report changes
• Assist with activities of daily living (ADLs)
• Document patient care accurately in EMR systems
• Collaborate with healthcare team members
• Support patient safety and infection control practices
• Float between units within the same facility as needed
