About The Position

Kaiser Permanente nurses are guided by an integrated nursing model that places patients and families in the center. The Licensed. Practical Nurse II (LPN II) - Health Education practices nursing as defined by the Georgia Board of Nursing standards of practice for Licensed Practical Nurses and by organizational policies, procedures, and guidelines. The LPN II - Health Education, under the supervision of the Registered Nurse or Clinician, provides direct and indirect nursing care to patients and families that prevent illness or injury, including complex procedures that restore health. Educates patients and families and assists with coordination of care. Collaborates with members of the Health Care Team (HCT) and leaders to ensure the delivery of high-quality care that meets defined standards that optimize health and wellness.

Responsibilities

  • Conducts community outreach and education sessions to raise awareness about the benefits of proactive health management.
  • Engages community organizations and public sector entities to promote health education initiatives and collaborative wellness programs.
  • Provides clinical services in a variety of settings (e.g. clinic, employer group sites, mobile health vehicle, etc.).
  • Ensures patient consent and other forms are complete and legible before offering services.
  • Prepares patients for the encounter by reviewing the medical history, collecting relevant clinical information, measuring vital signs, and documenting the medical record.
  • Completes POE and addresses care gaps during the encounter, to include scheduling appointments for patients with proactive care needs.
  • Promotes general preventative health measures, provides patient education in according to screening test guidelines, and refers patients to appropriate resources.
  • Ensures seamless coordination and communication between patients and healthcare providers for follow-up care.
  • Assists with event logistics and coordination, including inventory and supplies management, event tracking and reporting, and setting up and closing. Ensures proper tracking and storage of equipment and forms.
  • Utilizes advanced technologies for effective health screening, data collection and reporting, and patient tracking.
  • Participates in emergency care as directed by a clinician or registered nurse.
  • Provides departmental orientation and training to new team members on standard work, policies, and procedures.
  • Demonstrates the ability to be self-directed and act independently to prioritize multiple projects and tasks.
  • Assures team/departments compliance with regulatory agency requirements and federal, state, and local laws.
  • Participates in departmental or regional quality improvement activities and initiatives.
  • Leads one or more project(s) annually to improve access, clinical performance (e.g. HEDIS, CMS), and/or care experience.
  • May perform other duties as assigned.
