About The Position

The Licensed Practical Nurse (LPN) at the City of Chesapeake provides essential care and support to individuals with intellectual and physical disabilities. This part-time role involves working under medical supervision to implement health care objectives, collaborate with interdisciplinary teams, and ensure the well-being of residents. The LPN is responsible for medication administration, maintaining medical records, and participating in the development of care plans, while also engaging in active treatment and crisis management as needed.

Requirements

• Requires a diploma in practical nursing.
• Minimum of one year of related, full-time equivalent experience.
• Licensure as a Licensed Practical Nurse by the Commonwealth of Virginia or a reciprocal state.
• Certification in CPR, Multi-media First Aid, and Handle With Care.

Nice To Haves

• Experience working with the Intellectually Disabled population in a group home setting.

Responsibilities

• Dispense medications to residents and perform nursing tasks as directed by a physician.
• Serve as a liaison for residents with community physicians and other providers.
• Maintain inventory of supplies and medications under the direction of a supervisor.
• Develop and update nursing care plans and complete monthly assessments.
• Participate in quarterly and annual ISP reviews and collaborate with QIDP regarding changes to health care objectives.
• Maintain accurate resident electronic medical records.
• Assist with the development of special projects, groups, and policy development for the home.
• Participate in staff meetings and trainings.
• Provide oversight to ensure health and safety of all individuals in the environment.
• Identify and defuse crisis situations and provide care during emergencies.
