About The Position

The Licensed Practical Nurse I – Residential delivers quality health care services to youth in a residential based setting under the direction and supervision of the Registered Nurse (RN). This position performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care.

Requirements

  • Successful completion of an accredited LPN program required.
  • Minimum 1 year related work experience required.
  • Active, unrestricted LPN license in NYS required.
  • Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.
  • Basic Life Support (BLS) training within 60 days of hire and recertification annually.
  • Ability to recognize signs of current and emerging health needs of children/youth and address issues proactively and effectively communicate with children/youth and families to assure appropriate understanding of treatment, issues, and objectives.
  • Possess and maintain accurate knowledge of all regulatory and practice regulations and treatment protocols with special attention to HIPAA requirements.
  • Demonstrate the highest standards for ethical and professional conduct at all times and towards all youth and families.
  • Proficiency with computer-based applications in order to independently complete work and add/use data.
  • Demonstrate a thorough knowledge of services provided both by the Agency and by external service providers and maintain contacts as needed in order to efficiently access care for clients.

Responsibilities

  • Administer medications safely and accurately and per medication policy. Report any unusual patient reactions to a medication to the RN immediately.
  • Document progress notes in the electronic medical record and any other required system clearly, accurately, and timely meeting professional and agency standards to ensure accurate child and youth records are maintained.
  • Order and maintain inventory of all stock medications and medical supplies.
  • Administer basic first aid and CPR in emergency situations.
  • Provide ongoing medication, wellness and health education to youth.
  • Facilitate and assist in scheduling youth for appointments as directed.
  • Independently complete commitments and take responsibility for actions; recognize and fix errors and omissions.
  • Keep abreast of and adhere to all applicable safety regulations, including universal precautions and hazardous material safety rules.
  • Support process improvement initiatives in collaboration with leadership.
  • Understand and adhere to all federal, state, and local statutes, regulatory agency standards and Hillside policies including but not limited to all federal, state, and local statutes, regulatory agency standards, and Hillside policies, including but not limited to, Office of Mental Health (OMH), Office of Child and Family Services (OCFS), Council on Accreditation (COA) and New York State Education Department (NYSED).
