As an LPN you will have the opportunity to provide one on one patient care and utilize your clinical skills in providing end of life care as directed by an attending physician. Requirements

• Licensed Practical Nurse (LPN) with current license in the state of employment.
• Minimum one-year experience as an LPN in an acute care setting is highly recommended.
• Hospice experience is desired.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Perform care by following order sets and goals made by RNCM and notifying the RNCM of any changes in patient condition.
• Observe and monitor patient conditions and reconcile medications prescribed by the physician.
• Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Attend in-service training and mandatory agency meetings.
