About The Position

Requirements

  • License and current registration to practice as an LPN in New York State required.
  • High School Diploma or completion of LPN Program required.
  • Minimum of one year work experience as an LPN preferred.
  • Bilingual skills may be required based on operational needs.
  • Home care/hospice experience preferred.

Nice To Haves

  • Experience working in mental health and/or home care strongly preferred.
  • Cross-training in multiple service areas (Home Care, Hospice Residence, Crisis Care) is beneficial.

Responsibilities

  • Monitor and follow patient progress, reporting health changes to supervising registered professional nurse.
  • Deliver personalized nursing and care management by evaluating patient needs and adjusting care plans accordingly.
  • Coordinate care by communicating effectively within the VNS Health interdisciplinary team and with external providers.
  • Travel to patients' homes or care facilities, managing a weekly caseload with effective time management.
  • Provide continuity of patient care, coordinating supplies and offering education to patients and families.
  • Utilize technology to maintain accurate electronic medical records (EMRs) for patients in the community.
  • Educate and support patients and families in navigating end-of-life treatment options.
