Licensed Practical Nurse, Home Health
About The Position
Integrity Home Care + Hospice is seeking a compassionate Home Health Licensed Practical Nurse (LPN) to provide nursing care to patients in the comfort of their own homes in Lebanon, MO and surrounding areas. This full-time position aims to reduce stress and burnout by offering flexible schedules and 1:1 direct patient care. The role is supported by hands-on training, virtual learning, and 24/7 clinical support from an interdisciplinary care team. Requirements
- Active and current LPN license
- Ability to travel locally to cases as assigned
Nice To Haves
- One year of clinical experience preferred
- Previous home health care experience welcomed, but not required
Responsibilities
- Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization
- Case manage, develop care plans, and deliver highly skilled care – including assessments, medication management, wound care, and more
- Progress your clients from discharge to meeting their goals at home
- Provide education to the patient and/or their family
