About The Position

The Home Care Nurse (LPN) is responsible to provide nursing care consistent with the plan of care established by the RN Case Manager. This role involves delivering intermittent skilled nursing services to patients and their families, ensuring compliance with state and Medicare regulations, and maintaining documentation in accordance with agency policies. The nurse will work under the direction of the RN Case Manager and is expected to develop and maintain professional skills to meet patient care needs.

Requirements

• Graduate of NLN approved school of licensed practical nursing.
• Possession of a current license to practice as a licensed practical nurse in the State of Connecticut.
• One (1) year of recent medical/surgical nursing experience, additional experience in community health nursing preferred.
• Knowledge of professional nursing theory and practice.
• Skill in applying and modifying the principles, methods and techniques of professional nursing.

Responsibilities

• Provides intermittent skilled nursing service to patients and their families in compliance with state and Medicare regulations.
• Assesses the physical and psychosocial status of the patient.
• Follows through on an appropriate care plan and therapeutic goals with the patient/family and RN Case Manager.
• Refers to appropriate therapies with approval of the RN Case Manager.
• Conducts periodic reviews of progress toward therapeutic goals in conference with the RN Case Manager.
• Documents all services provided in the clinical record in accordance with state and federal regulations and Agency policy.
• Confers regularly with the RN Case Manager assigned.
• Maintains required productivity as established by the Agency.
• Attends a minimum of 12 hours of in-service per year.
• Maintains CPR certification.
• Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan.
