The Licensed Practical Nurse (LPN) for Hospice Services is responsible for delivering nursing services to Hospice Services patients as prescribed by a physician, ensuring compliance with the state's Nurse Practice Act, Federal Regulations, and other regulations defining Georgia’s Hospice Aide services. The LPN provides a range of services to meet patients' personal care needs and promote their comfort and dignity. Requirements

  • Ability to effectively communicate and present information in a clear and concise manner, both orally and in writing.
  • Transports self to patient's home/Facility, may be in multiple locations in a workday
  • Ability to establish and maintain effective working relationships with associates, department heads, administrators, member organization presidents and their leadership teams, and the general public.
  • Nursing degree from an accredited college or university or is a graduate of an approved LPN program
  • Current license / registration by state of hire as a LPN.
  • Current CPR certification.

Responsibilities

  • Provides direct patient care as defined in the LPN State Nurse Practice Act
  • Implements current nursing practice following a patient assessment and an approved Plan of Care initiated by the Hospice Services Registered Nurse.
  • Provides accurate and timely documentation of Hospice patient services to reflect the Plan of Care.
  • Assesses and provides Hospice Services patient/family/caregiver education and information pertinent to diagnosis and Plan of Care.
  • Participates in coordination of agency services, appropriately reporting the identified needs for agency Interdisciplinary Group members.
  • Uses equipment and supplies effectively and efficiently.
  • Provides appropriate pain/symptom management; evaluates Hospice Services patient’s response(s) to treatments/medications.
  • Participates in personal and professional growth and development.
  • Participates in the organization’s quality management program.
  • Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
  • Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
  • Performs other duties as necessary to ensure the success of the System.
