Responsible for providing skilled nursing services to Home Health Services patients and as prescribed by the physician to be in compliance with the state's Nurse Practice Act, Federal Regulations and any other regulations defining Georgia's home health services. The Licensed Practical Nurse (“LPN”) is responsible for the provision of a number of services to meet the patients' nursing needs and to promote their comfort and dignity.

Responsibilities

  • Provides direct patient care as defined in the LPN State Nurse Practice Act.
  • Participates in the Home Health Services Interdisciplinary Group meetings as appropriate.
  • Implements current nursing practice following a patient assessment and an approved Plan of Care initiated by the Registered Nurse (“RN”).
  • Collaborates with the RN to review/revise the plan of care, including a discharge plan, based upon the assessment of patient needs.
  • Provides accurate and timely documentation of patient services to reflect the Plan of Care.
  • Assesses and provides patient/family/caregiver education and information pertinent to diagnosis and Plan of Care.
  • Participates in coordination of Home Health agency services, appropriately reporting the identified needs for agency Home Health Services Interdisciplinary Group members.
  • Uses equipment and supplies effectively and efficiently.
  • Participates in personal and professional growth and development.
  • Participates in Home Health Services Quality Management Program.
  • Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
  • Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
  • Performs other duties as necessary to ensure the success of the System.

Requirements

  • Nursing degree from an accredited college or university or is a graduate of an approved LPN program.
  • Current license / registration by state of Georgia as an LPN.
