About The Position

Viva Senior Living at Greenpoint is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Enriched Housing Program. In this vital role, you will provide hands-on nursing care, monitor residents’ health status, and support their ability to live independently in a warm, community-focused environment. Requirements

• Must possess a valid LPN license issued by the State.
• Must possess, at a minimum, a high school diploma or a GED.
• Must be able to work well with peers in a congenial work environment.
• Must maintain the utmost compassion and dignity to the residents and adhere to the Resident Bill of Rights.
• Must adhere to Viva Senior Living’s attendance policies.
• Must be able to read, write, speak and understand English.
• Must provide annual verification of a negative TB skin test.
• Proof of full COVID-19 vaccination required with exceptions for medical and religious accommodations.

Responsibilities

• Provide care, treatment, medication administration to assigned residents, and continually monitor and assess the residents’ condition.
• Assist with oversight of ancillary care staff in the absence of the Wellness Director or Assistant Wellness Director.
• Provide care to assigned residents based on plans of care.
• Medication assistance and/or administration with adherence to the Five Rights of Medication Administration.
• Reorder and receive medications.
• Provide personal care and ADL assistance.
• Supervise Care staff.
• Maintain shift to shift report.
• Provide emergency assistance to residents with appropriate follow-up.
• Obtain and transcribe physician orders.
• Maintain case management documentation.
