About The Position

Recruit Secure is seeking an experienced Licensed Home Care Services Agency (LHCSA) Coordinator to support an Assisted Living Program (ALP). This role is responsible for overseeing home care operations, medical record management, and HHA/PCA coordination in accordance with State, Federal, and local regulations . The LHCSA Coordinator reports directly to the Director of Patient Services and plays a critical role in ensuring quality care delivery and regulatory compliance.

Requirements

  • Active New York State Licensed Practical Nurse (LPN) license (required)
  • Prior experience in home care/alp settings (required)
  • Minimum of two (2) years nursing experience
  • Minimum of two (2) years administrative and/or supervisory experience

Responsibilities

  • Oversee daily operations and management of the Agency/ALP
  • Manage facility medical records and HHA/PCA services
  • Communicate with CHHA regarding skilled services and changes in resident condition requiring assessment
  • Ensure adequate staffing levels with properly trained and experienced personnel
  • Maintain compliance with all local, state, and federal rules, regulations, and statutes
  • Participate in the development and implementation of administrative policies and procedures
  • Support quality assurance, operational efficiency, and regulatory readiness
