About The Position

The LPN Clinical Coordinator (LPNCC) plays a vital role in supporting our clinical field staff and Clinical Supervisor by managing staff assignments, coordinating schedules, and providing hands-on clinical and administrative support. This is a full-time, office-based position located at our Syracuse Headquarters, with regular hours Monday through Friday, 8:00 AM to 4:30 PM.

Requirements

  • Graduate of an accredited licensed practical nursing program with current New York State Licensed Practical Nurse registration.
  • This role is required to report in person to the Nascentia organizational office Monday-Friday 8am-4:30pm located in Syracuse, NY.
  • Skills necessary include excellent computer, verbal, and written communication skills.
  • Excellent organizational skills with the ability to prioritize multiple tasks.
  • Excellent customer service both internal and external customers.
  • Able to communicate effectively in English, both verbally and in writing.
  • Ability to sit or stand at a desk 90% of the day
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to express self verbally and in writing
  • Ability to use telephone and communicate over the phone
  • Strong computer literacy and comfort, ability and desire to advance technically and understand industry standards.
  • Proven analytical and problem-solving skills to identify potential issues and participate in their resolution.
  • Proficiency with computer systems, working knowledge of Microsoft Office, Internet, and various software applications.
  • Demonstrate the ability to work independently and collaboratively with the ability to adapt to shifting priorities, demands, and timelines.
  • Demonstrates discretion in handling confidential and sensitive information
  • Excellent communication skills, verbal and written.
  • This role is required to report in person to a Nascentia organizational office Monday- Friday 8am-4:30pm.
  • Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids

Responsibilities

  • Provides administrative and technical support to the Clinical Supervisor and clinical staff.
  • Responsible for maintaining availability of employees in scheduling system.
  • Responsible for supporting the clinicians, Clinical Supervisor in the initial and ongoing scheduling of the patient visits for all disciplines including onboarding clinicians.
  • Completes reconciliation of schedules for assigned team/clinicians/HHA supervisions.
  • Coordinates with intake staff and other departments for efficient and timely initiation of services.
  • Intake of telephone calls and other clinical duties as assigned.
  • Document accurately and completely as required.
  • Track and disseminate information to appropriate department(s) regarding patients in facilities.
  • Participates in departmental educational in-service and team planning sessions.
  • Participates in appropriate agency meetings and quality improvement and utilization review activities of the organization.
  • Responsible for obtaining ongoing authorizations as required.
  • Participates in special projects and performs other duties as requested.
