Job Overview: As an LPN Case Manager, you will be responsible for assessing, planning, implementing, reevaluating and revising care and coordinating resources for program participants. You will update and maintain patient records, implement orders for medications and treatments and perform assessments. You will also ensure ongoing education, training, and professional development of all staff that is relevant to care of program participants. New College Grads Welcome to Apply! This role is a part-time role at 24 hours per week.

Requirements

  • Willingness to travel within assigned region required.
  • Valid driver’s license and reliable transportation required.
  • Licensed as LPN by Massachusetts Board of Registration in Nursing.
  • CPR Certification.
  • Strong communication, documentation, and organizational skills are required.
  • Must be solution oriented and be able to multitask.

Nice To Haves

  • Fluency in both English and second language strongly preferred.
  • Minimum of one year experience in a skilled nursing or home care setting or comparable work experience preferred.
  • Case management, behavioral health, social services and/or community health worker experience preferred.

Responsibilities

  • Complete monthly, in-home visits, caregiver education/skills training and assess health and environmental changes.
  • Conduct semi-annual visits, reviewing and updating existing plan of care and corresponding goals, medication review and reconciliation and review caregiver communication tools.
  • Address concerns regarding environment, psychosocial status and changes in physical health.
  • Participate in multidisciplinary team meetings and work collaboratively to address client needs.
  • Complete all required documentation per regulation and communicate professionally with client PCP and other healthcare providers when necessary.
  • Serve as a brand ambassador for VITRA reflecting our vision, mission, and values.
  • Show a genuine interest and compassion for the communities we serve and commitment to the diversity of our clients and team members.
  • Mentors and supports team members.
