Vitra Health is hiring a Part-Time Licensed Practical Nurse (LPN) to support clients in Lynn and its surrounding areas, including Salem, Peabody, and North Reading. As an AFC provider, Vitra Health's Adult Foster Care (AFC) program is funded by Mass Health and provides stipend payments to qualified caregivers assisting members in need of care. The LPN will work alongside a Nurse and Case Manager to provide essential support to clients and caregivers. This role involves educating clients and caregivers on health maintenance, assisting with healthcare management navigation, and accessing community support. New college graduates are welcome to apply.
