Hines & Associates is seeking nurses who are ready to transition from hands-on nursing to a role that allows for a better work-life balance. This position offers no nights, weekends, or holidays, and provides the opportunity to work from home 50% of the time after training. The role involves utilizing medical knowledge to assess patient needs and develop care plans to help patients achieve optimal health. Responsibilities include telephonic assessment, care planning, implementation, coordination, monitoring, evaluation, and outcomes measurement. The LPN care coordinator acts as a patient advocate, promoting safety and maintaining privacy and confidentiality in accordance with policies and procedures. This is a full-time position, Monday to Friday, with an in-office requirement until the completion of the orientation period, after which a hybrid work option is available.
