About the position

Charter Oak Health Center is seeking a Bilingual Licensed Practical Nurse (LPN) to join our team. In this role, you will be responsible for providing compassionate, patient-centered care. The ideal candidate will have a strong background in supporting clinical operations by delivering high-quality nursing care, promoting health education, and ensuring effective communication with diverse patient populations. This role involves assisting with patient care, monitoring vital signs, and ensuring patient comfort. The LPN must be proficient in both English and Spanish to effectively assist patients with limited English proficiency. Responsibilities

  • Provide direct nursing care under the supervision of a registered nurse (RN) or healthcare provider.
  • Perform clinical tasks such as vital signs assessment, wound care, medication administration, and injections.
  • Assist in managing chronic conditions such as diabetes, hypertension, and asthma.
  • Collect patient histories, document symptoms, and prepare patients for examinations.
  • Support providers during medical procedures and exams.
  • Administer immunizations following state and federal guidelines.
  • Serve as a medical interpreter to facilitate clear communication between patients and healthcare providers.
  • Translate educational materials, discharge instructions, and consent forms to ensure patients understand their care plan.
  • Assist in coordinating care and connecting patients to community resources.
  • Accurately document patient encounters, treatments, and assessments in the Electronic Health Record (EHR) system.
  • Ensure proper documentation aligns with HRSA, OSHA, and FQHC compliance standards.
  • Collaborate with multidisciplinary teams to ensure continuity of care.
  • Provide culturally appropriate health education on topics such as medication adherence, disease prevention, and wellness strategies.
  • Educate patients and families on treatment plans, preventive care, and self-management strategies.
  • Follow infection control protocols and maintain a clean, organized clinical environment.
  • Ensure compliance with OSHA, HIPAA, and FQHC standards.
  • Participate in quality improvement initiatives to enhance clinical outcomes.

Requirements

  • Completion of an accredited LPN program.
  • Active Licensed Practical Nurse (LPN) license in the State of CT.
  • Experience in a clinical setting is preferred but not always required for entry-level positions.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Proficiency in basic nursing procedures and patient care techniques.
  • Attention to detail and strong organizational skills.
  • Certification in Basic Life Support (BLS) is often required.
