About The Position

The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran. The(LPN/LVN) also performs assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving the veteran. The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran. The(LPN/LVN) also performs assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving the veteran. The (LPN/LVN) is considered a novice and performs within this functional statement under close supervision of the Nurse Manager or designee. Assigned tasks require basic technical competence and skill. The duties include but not limited to: The Licensed Practical Nurse works under the direction of an RN. The LPN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic life care plan. The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies, and procedures. The primary Care LPN is a member of the Core Team, working as a Clinical Associate (CA). The CA demonstrates active collaboration and communication with the Core Team (Patient, Primary Care Provider, RN Care Coordinator and Clerk), the Therapeutic Monitoring Clinic (TMC) and the Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). Adheres to standards of practice and safe technique in preparation and administration of medication. Processes knowledge of proper handling and and pre-cleaning of reusable medical equipment. Demonstrates competence utilizing special equipment needed to perform job duties.
