About The Position
Home Health Care Inc. is searching for skilled individuals like you to join our team on a per diem basis, providing exceptional care to clients across the Twin Cities area. As a valued member of our team, you'll provide treatment visits to clients in their homes or within Senior Living Communities. You will implement treatment plans established by the physical therapist, delivering interventions and home exercise programs, and instruct patients on physical therapy interventions and modalities, fostering excellent patient outcomes.
