About The Position
M.D. Home Health, LLC is seeking a Licensed Physical Therapist to provide in-home care throughout the Lake Worth Area or Boynton Beach to Delray Area. This is a flexible, patient-centered opportunity for a clinician who values autonomy, efficiency, and meaningful patient impact. You won’t be stuck in a clinic juggling 3 patients at once—you’ll deliver one-on-one care where it actually matters: in the patient’s home. BONUS OPPORTUNITY: Earn up to $1,500 in performance-based bonuses for maintaining a consistent caseload, with additional incentives for servicing high-demand areas.
Requirements
- Active Florida Physical Therapist License (Required)
- Minimum 1 year of clinical experience (home health preferred, but not required)
- Strong clinical decision-making and time management skills
- Ability to work independently in the field
- Reliable transportation and willingness to travel within assigned coverage area
- Comfortable with EMR documentation
- CPR certification

Responsibilities
- Perform in-home evaluations and reassessments to develop individualized plans of care
- Deliver skilled therapy interventions to improve mobility, strength, balance, and independence
- Provide fall prevention, gait training, and functional mobility training
- Educate patients and caregivers on home exercise programs and safety strategies
- Communicate with physicians and interdisciplinary team members to ensure coordinated care
- Complete documentation in a timely manner using our EMR system
- Manage your caseload efficiently within your assigned territory
