About The Position

Relient Health is seeking licensed Physical Therapists (PTs) for full-time, in-person opportunities within established rehabilitation departments across New Hampshire. These are full-time, direct-hire, permanent placement PT positions (not travel, not contract, not PRN). Our openings include opportunities in Skilled Nursing Facilities (SNFs), acute care hospitals, inpatient rehab facilities (IRFs), outpatient clinics, and home health settings depending on the specific location. 📍 Current PT openings available in: Bedford, Concord, Franconia, Franklin, Hampton, Laconia, Lancaster, Lebanon, Peterborough, Unity, Winchester, Wolfeboro, and additional New Hampshire markets. 🔎 Who Are These Opportunities Ideal For? Licensed New Hampshire Physical Therapists PTs relocating to New Hampshire New graduates from accredited DPT programs pursuing New Hampshire licensure PTs seeking experience in SNF, hospital/acute care, IRF, outpatient, home health, or mixed-setting rehab roles This role is specific to licensed Physical Therapists working within a rehabilitation department. This is not a CNA, nursing assistant, medical assistant, or therapy aide position.

Requirements

  • Graduate of an accredited Doctor of Physical Therapy (DPT) program
  • Active New Hampshire PT license, or eligibility/pending licensure
  • Ability to work full-time in one of the listed New Hampshire markets
  • Authorized to work in the United States

Responsibilities

  • Evaluate patients and develop individualized treatment plans
  • Provide skilled physical therapy services in a skilled nursing, hospital/acute, inpatient rehab (IRF), outpatient, or home health environment
  • Collaborate with interdisciplinary rehab and nursing/medical teams
  • Supervise and partner with Physical Therapist Assistants (PTAs)
  • Document care in EMR systems per state, federal, and facility standards
  • Promote functional independence, safety, and quality of life
