Requirements

Current Licensed Physical Therapist Assistant (LPTA) in Arizona or license eligible.
Associate’s degree in Physical Therapist Assisting from an accredited program.
Strong communication and interpersonal skills.
Self-motivated with the ability to manage a flexible schedule.
Passion for home health and community-based care.
Strong assessment, diagnostic, and therapeutic abilities.
Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems.
Proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

Assist in implementing treatment plans designed by Physical Therapists.
Provide direct patient care including therapeutic exercises, manual therapy, and modality applications under the direction and supervision of a Physical Therapist.
Deliver compassionate, individualized care in patients’ homes.
Monitor and accurately document patient progress, treatments, and responses in compliance with facility policies and regulatory standards.
Communicate effectively with the interdisciplinary team and report observations to the Physical Therapist.
Educate patients and caregivers/families on treatment protocols, exercises, and self-care techniques to promote functional independence.
Travel throughout Maricopa County to serve assigned patients.
Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
