Requirements

  • Current Licensed Physical Therapist Assistant (LPTA) in Arizona or license eligible.
  • Associate’s degree in Physical Therapist Assisting from an accredited program.
  • Strong communication and interpersonal skills.
  • Self-motivated with the ability to manage a flexible schedule.
  • Passion for home health and community-based care.
  • Strong assessment, diagnostic, and therapeutic abilities.
  • Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems.
  • Proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

  • Assist in implementing treatment plans designed by Physical Therapists.
  • Provide direct patient care including therapeutic exercises, manual therapy, and modality applications under the direction and supervision of a Physical Therapist.
  • Deliver compassionate, individualized care in patients’ homes.
  • Monitor and accurately document patient progress, treatments, and responses in compliance with facility policies and regulatory standards.
  • Communicate effectively with the interdisciplinary team and report observations to the Physical Therapist.
  • Educate patients and caregivers/families on treatment protocols, exercises, and self-care techniques to promote functional independence.
  • Travel throughout Maricopa County to serve assigned patients.
  • Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
