About The Position

Hartford HealthCare at Home is looking for a Physical Therapist Assistant to work under the clinical supervision of a Physical Therapist. The role involves administering Physical Therapy treatments to patients and assisting the Physical Therapist in evaluating patients' levels of function and determining treatment strategies. Requirements

  • Associates Degree from an accredited Physical Therapy Assistant program.
  • Current State of CT Physical Therapy Assistant license required.
  • Valid driver’s license and reliable transportation required for Home Care.

Responsibilities

  • Assists the Physical Therapist with aspects of patient assessment by performing delegated duties such as testing for joint range of motion, muscle strength, and mobility status, identifying rehab precautions.
  • Recognizes physiological and age-related changes in the patient and consults with the Physical Therapist and other Health Care Team members.
  • Performs therapeutic techniques appropriate to patient’s needs and medical status under the clinical supervision of the Physical Therapist.
  • Implements treatment plans provided by the Physical Therapist and delivers patient care using appropriate interventions.
  • Communicates patient progress to the Physical Therapist.
  • Provides patient and caregiver education regarding treatment, precautions, and equipment use as delegated by the Physical Therapist.
  • Completes all documentation including evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan.
  • Assists with patient transition planning and/or discharge planning including recommendations to community resources.
