About The Position

We are seeking a Licensed Occupational Therapist Consultant to support a one-year grant-funded home modification program serving older adults across the five boroughs of New York City. This part-time role focuses on fall prevention, home safety assessments, accessibility improvements, and coordination of repair and modification services to promote independent living for low-income seniors. The Consultant will split time between remote administrative/outreach duties and in-person home visits to conduct assessments and oversee modifications.

Requirements

  • Valid New York State license to practice Occupational Therapy.
  • Bachelor’s degree in Occupational Therapy from an accredited institution recognized by the U.S. Secretary of Education and the Council for Higher Education Accreditation.
  • Minimum three (3) years of satisfactory full-time occupational therapy experience under professional supervision.
  • At least one (1) year of experience in an educational setting.
  • Ability to conduct in-person home visits throughout NYC.
  • Must have a personal cell phone and computer (desktop or laptop) with Wi-Fi access (minimum 200 Mbps internet speed).
  • Equipment capable of supporting virtual meetings and trainings (webcam, microphone, headset, and required technical specifications).

Nice To Haves

  • Bilingual (Spanish, Chinese, or Russian) highly preferred.
  • Experience working with low-income older adult populations strongly preferred.
  • Strong organizational, problem-solving, and decision-making skills.
  • Excellent written and verbal communication skills.
  • Ability to work independently with minimal supervision.
  • Proficiency in Microsoft Office Suite and virtual meeting platforms (Zoom, WebEx, Facebook Live, etc.).

Responsibilities

  • Outreach & Referral Coordination Participate in outreach activities, including meetings, site visits, and presentations to provider agencies, hospital discharge planners, homecare staff, and community organizations.
  • Deliver in-person and virtual presentations outlining program services and referral processes.
  • Review new referrals weekly, determine program eligibility, and communicate acceptance or denial to referral sources and clients.
  • Maintain strong communication with referral networks and respond promptly to inquiries or concerns.
  • Client Assessment & Home Modification Oversight Conduct in-person home visits across NYC’s five boroughs to assess fall risk, safety, accessibility, and functional limitations.
  • Research and recommend home modifications, durable medical equipment, accessibility tools, and limited repairs tailored to client needs.
  • Collaborate with the Project Director and Home Modification Contractor to oversee service contracts and ensure timely completion of work.
  • Conduct follow-up visits to ensure repairs and modifications are functioning effectively and supporting daily living activities.
  • Provide training to clients on the proper use of installed equipment and modifications.
  • Data Management & Reporting Coordinate intake documentation and ensure accurate entry into the client tracking database.
  • Track timelines, assessment completion, and project metrics.
  • Assist with preparing reports for funding organizations.
  • Support the development of sustainability plans, including documentation of referral networks, policies, procedures, best practices, and outcome measures.
  • Help develop and maintain a resource library related to grant implementation.
  • Team Collaboration Participate in regular team meetings to address issues, identify opportunities, and develop action plans.
  • Maintain consistent communication with team members and grant participants.
